Description
HCR ManorCare provides a range of services, including skilled nursing care, assisted living, post-acute medical and rehabilitation care, hospice care, home health care and rehabilitation therapy.
The MDS Coordinator will assist the Director of Nursing (DON) with ensuring that documentation in the center meets Federal, State, and Certification guidelines. The MDS Coordinator will coordinate the RAI process assuring the timeliness, and completeness of the MDS, RAPS, and Interdisciplinary Care Plan.

Position Requirements
Minimum of 2 years of nursing experience in a Skilled Nursing Facility preferred. Excellent knowledge of Case-Mix, the Federal Medicare PPS process, and Medicaid reimbursement, as required. Through understanding of the Quality Indicator process. Knowledge of the OBRA regulations and Minimum Data Set. Knowledge of the care planning process.
Educational Requirements
Graduate of an approved Licensed Practical Nurse program and licensed in the state of practice required.
