Physical Therapist - Home Health & Hospice
Description
HCR ManorCare provides a range of services, including skilled nursing care, assisted living, post-acute medical and rehabilitation care, hospice care, home health care and rehabilitation therapy.
Responsible to provide medically prescribed, goal oriented therapy in accordance with the Physician Plan of Care, company policies, and professional Standards of Practice.

Position Requirements
100% travel necessary on a daily basis. At least one year or more work experience in physical therapy in a multidisciplinary setting is preferred.
Knowledge of principles, concepts, and methodology of physical therapy appropriate for terminally ill patients.
Excellent communication skills, and ability to develop and maintain rapport with clients and families.
Educational Requirements
Bachelor's or Master's degree in physical therapy from an accredited institution. Currently licensed as a Physical Therapist in the state and in good standing with the Board.
