Medical Social Worker

Description
HCR ManorCare provides a range of services, including skilled nursing care, assisted living, post-acute medical and rehabilitation care, hospice care, home health care and rehabilitation therapy.

Responsible to the Director of Clinical Services/Patient Care Coordinator to provide clients and families psychosocial care regarding economic, social, and emotional needs.

*Assess client/family factors to estimate capacity to cope with health situation.

*Directs and supervises Social Worker in care given to client.

*Evaluates client's insurance coverage and pursues options for assistance if needed.

Position Requirements
Prefer one year experience in a health care setting.

100% travel necessary on a daily basis.

Demonstrate problem solving and decision making skills.

Demonstrated ability to work will with an interdisciplinary team.

Excellent communication skills.

Ability to develop and maintain rapport with the client and family.

Educational Requirements
Masters Degree in Social Work from an accredited institution.

May require licensure or certificate as required by the state, and in good standing. (if applicable)
