Heartland Home Health Care, a national, progressive Home care provider has immediate opportunities for full-time, part-time or per diem Physical Therapists to support the Milwaukee, Waukesha, Washington, Ozaukee and surrounding areas. Our Physical Therapists participate in the development and implementation of the client's physical therapy plans of care and provide education to team members, clients and families in their homes.  In addition you will experience:

100% travel necessary on a daily basis. At least one year or more work experience in physical therapy in a multidisciplinary setting is preferred.
Knowledge of principles, concepts, and methodology of physical therapy appropriate for terminally ill patients.
Excellent communication skills, and ability to develop and maintain rapport with clients and families.

Bachelor's or Master's degree in physical therapy from an accredited institution. Currently license
